Please familiarize yourself with our 2023 policies and check the boxes below to adhere to these policies:

Daycare is $30.00 per day. Puppies and adult dogs on a case-by-case basis are charged $35.00 per day until the dog is acclimated to our program. There is a $5 clean up fee for dogs who have frequent accidents inside.
Daycare Hours are 8am - 6pm Monday through Friday. I agree to pay a $10 early drop off fee if my dog is dropped off before 8am and a late fee starting at $15 if I pick my dog up after 6pm. There is no grace period for early drop-off or late pickups.
I understand Go Play! is closed daily for lunch between 12pm - 1pm to allow our dogs to rest uninterrupted. My dog will not be able to be dropped off or picked up during this time.
I must cancel same-day daycare by 7:30am to avoid be charged the full daycare rate.
I understand I am responsible to pay 50% of my regular schedule to hold my permanent spot if my dog is absent for daycare for any reason. This fee will not be owed for days Go Play! is closed.
Go Play! requires a 2-week written notice of permanent schedule changes to avoid being charged in full for 2 weeks of the permanent schedule.
I understand that if I cancel my dog's permanent daycare services frequently, I am in danger of being removed from the permanent schedule with a 2-week notice provided by Go Play!
I understand my dog must be kept current on distemper / parvo / rabies vaccinations and have a yearly fecal flotation. If my dog is not current on these things they will not be able to participate in daycare until they are current.
Payment is due upon completion for services. Daycare fees are due on the last day of the week your dog attends daycare. I agree to pay a $10.00 per day late fee on any overdue bills.
I am responsible for providing my dog an identification (name) tag located on the collar / harness of my dog.
I understand Go Play! is closed the following 2023 dates: 5/29, 5/30, 5/31, 6/1, 6/2, 7/3, 7/4, 7/5, 7/6, 7/7, 9/1, 9/4, 9/22, 10/13, 11/23, 11/24, 12/25, 12/26, 12/27, 12/28, 12/29, 1/1/24.
I agree to keep my dog home if they are unwell (vomiting, diarrhea, lethargic, etc.).
I understand I should not feed my dog within 1-hour of dropping off for daycare to allow for digestive rest.

15-minute group walk or 10-minutes solo walk during daycare for $10.00
Monthly Easy Keeper Program: twice monthly your dog will receive 5-minutes of brushing, nail trimming, ear cleaning, teeth brushing, and cologne spray (unscented wipes available) ) - $40.00 charged monthly at the beginning of each month.
Unlimited Easy Keeper Program: everything included above, but done weekly - $70.00 charged monthly at the beginning of each month.
Once monthly nail trim - $20.00
Nail Care Package (twice monthly nail trimming / grinding) $30.00 monthly, charged at the beginning of each month.
Grooming Package - allow us to schedule your dog's grooming (includes bath, nail trimming, ear cleaning, teeth brushing, and haircut if applicable) while they are at daycare every 4 - 8 weeks to receive $10.00 off your normal grooming price.
By checking this box and submitting this form, I agree all the information I have shared is accurate. I also give Go Play! permission to contact my veterinarian to verify my dog is current on the required vaccinations and fecal flotation needed to participate in daycare. Go Play! (hereafter referred to as Company) is entrusted to use best judgement in caring for Client's animals. In the event of an emergency, inclement weather, natural disaster, or other circumstances beyond all control prohibit Company from caring for animals, Company will be held harmless for related consequences. Company has all rights to refuse services due to inclement weather or other dangerous circumstances. Client is responsible for injury to persons, animals, property, death, or injury to animals named in this contract. Client agrees to pay any veterinary costs to animals during time they are being cared for by Company. If animals become ill, the Company may engage the services of a veterinarian to administer medicine or provide medical care to animals. Company or representative is bitten, injured, or exposed to any disease or ailment received from Client's animals it is the Client's responsibility to pay all costs and damages incurred. Company has the right to refuse services to any animals for any reason. Company agrees to use Client's equipment for animals and is not responsible for faulty, damaged, or damages to equipment. Client understands pet grooming includes the use of sharp tools and clippers and the difficulty of using those tools on live animals. Client understands there is a risk of clipper / brush irritation, eye, ear, and skin irritations due to a matted, dirty, or unkept coat and grooming. Company is released of all faults when providing grooming services. If any circumstances arise, leading to additional charges upon an agreed price, Client is responsible for additional charges. Company will have printed copy of this contract available to Client. This is a valid and binding contract for all services provided by Company.
